Psychiatric notes are a way to reflect on your practicum experiences and connect them to the didactic learning you gain from your NRNP courses. Comprehensive psychiatric evaluation notes, such as the ones required in this practicum course, are often used in clinical settings to document patient care.

For this Assignment, you will document information about a patient that you examined in a group setting during the last 4 weeks, using the Comprehensive Psychiatric Evaluation Note Template provided. You will then use this note to develop and record a case presentation for this patient.

 
To Prepare

Select a group patient for whom you conducted psychotherapy for an impulse control or conduct disorder during the last 6 weeks.

In your presentation:

The Assessment Brief

A presentation of 20 minutes duration which may be delivered on-line or in person. Using an example from practice, demonstrate your understanding of your role in the care of an individual with two or more health issues or diagnosed health conditions. This should also consider the impact of your role on the individual and others who may be involved.

Requirements of assessment to meet all learning outcomes

Identify a suitable patient / service user on which to base your presentation. Your selected patient will need to be an individual that you are actively involved with planning and delivering care to in your role as a student nurse. If this is not possible due to Coronavirus restrictions or because you have opted out of PLP 1 due to the recognition of prior learning (RPL), then please refer to and choose one of the scenarios provided by the module team.

 

Your selected patient will need to have two or more identified health issues and or diagnosed health conditions

Based upon your chosen patient/service user you need to consider and discuss each of the following within your presentation in order to meet the learning outcomes of the module

 

Ø Professional issues relating to the patient,

  • Links made to the Nursing and Midwifery Council (NMC) professional code and standards throughout
  • Confidentiality must be considered and stated within the work

 

Ø The health issues/diagnosed health conditions

  • Provide the context of the individual – to include their background, their social circumstances, events which brought them under your care.
  • Consideration of physical, emotional, mental, financial impact and the effects on family and carers in this episode of care.
  • Brief overview of each health issue / diagnosed health condition, there must be a minimum of two

Being a doctor in the 21st century is not just about diagnosing and treating patients, but a commitment to improve the quality of care provided. Improving quality is about making health-care safe, effective, patient-centered, timely, efficient, and equitable. The terms used around ‘quality improvement’ can be confusing, and this section will provide details of these terms, and an overview of the processes involved in quality improvement.

What is quality?

 

Within healthcare, there is no universally accepted definition of ‘quality’. The United States (US) Institute of Medicine defines quality as ‘the degree to which health services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge(US Institute of Medicine, 2001). The Institute of Medicine has identified six dimensions of healthcare quality:

 

 

1. Safe – Avoiding harm to patients from care that is intended to help them.
2. Effective – Providing services based on evidence and which produce a clear benefit.
3. Person-centered – Establishing a partnership between practitioners and patients to ensure care respects patients’ needs and preferences.
4. Timely – Reducing waits and sometimes harmful delays.

What is Quality Improvement?

 

There is no single definition of quality improvement. However a number of definitions describe it as an evidence-based systematic approach that uses specific techniques to improve the quality of processes of care for measurably better outcomes. All good organisations continually review and modify their processes to improve the quality and outcomes of patient care.

 

These techniques share some simple underlying principles:

 

  • Understanding the problem with a particular emphasis on what the data tells you
