  1. Demonstrate knowledge and understanding of a range of health issues encountered when caring for people.

 

  1. Demonstrate an understanding of the student nurse’s role in safeguarding service users, their carers, families and the wider public.

 

 

The Assessment Brief

A presentation of 20 minutes duration which may be delivered on-line or in person. Using an example from practice, demonstrate your understanding of your role in the care of an individual with two or more health issues or diagnosed health conditions. This should also consider the impact of your role on the individual and others who may be involved.

 

Submission details

There are two ways in which you can present your work

 

A: A presentation in person at your campus base site (Coronavirus restrictions allowing)

OR

C: Narrated Power point presentation

 

You can choose which method of submission suits your learning needs; you are required to inform the module team which method you wish to submit your work by the formative submission date

 

All students must submit their summative presentation via Blackboard Assessment portal by the date stipulated in your module handbook – this will be prior to your presentation date.

 

It is important that throughout the presentation that you reflect upon your role as a student nurse and the impact it has upon the process described.

You are required to support your work with reference to appropriate, relevant and up to date authoritative literature. Support the actions taken and care provided with links to the literature, this is an important aspect of the assessment, there needs to be evidence that reading of up to date literature has influenced the assessment and care delivery of your identified patient/service user

Requirements of assessment to meet all learning outcomes

Identify a suitable patient / service user on which to base your presentation. Your selected patient will need to be an individual that you are actively involved with planning and delivering care to in your role as a student nurse. If this is not possible due to Coronavirus restrictions or because you have opted out of PLP 1 due to the recognition of prior learning (RPL), then please refer to and choose one of the scenarios provided by the module team.

 

Your selected patient will need to have two or more identified health issues and or diagnosed health conditions

Based upon your chosen patient/service user you need to consider and discuss each of the following within your presentation in order to meet the learning outcomes of the module

 

Ø Professional issues relating to the patient,

  • Links made to the Nursing and Midwifery Council (NMC) professional code and standards throughout
  • Confidentiality must be considered and stated within the work

 

Ø The health issues/diagnosed health conditions

  • Provide the context of the individual – to include their background, their social circumstances, events which brought them under your care.
  • Consideration of physical, emotional, mental, financial impact and the effects on family and carers in this episode of care.
  • Brief overview of each health issue / diagnosed health condition, there must be a minimum of two
  • Identify the impact these health issues / diagnosed health condition have upon the individual and their daily life

 

Ø The care needs – Discuss and critically analyse how care was planned for this patient and their role within this care (scope of practice).

 

  • The assessment of care needs – How were these care needs assessed? Nursing model? Which one? Demonstrate how this was used to assist with the assessment of your patient’s care needs
  • Were risk assessment tools used, which ones and why or if not why not?
  • Planning the delivery of care needs – How were these planned, care plan, pathway? Look at the strengths and limitations of the chosen approach.
  • Consideration of others in the assessment and delivery of the care needs (consider the patient, their family and carers, wider MDT)
  • Were these planned in junction with other professionals, for example Nurses, Physio, Doctors
  • How were the care needs reassessed and are provided evaluated?
  • Always link back to your role as a student nurse

 

Identifying and raising concerns as a student nurse – Evidence through care planning or discussion of the student nurse role in safeguarding patients, carers, families and the wider public

  • Recognising potential hazards – Were there any hazards recognised, even if not how was this assessment made
  • Recognising potential safeguarding concerns – Were there any safeguarding issues identified?
    • If yes how was the assessment carried out and what actions were taken upon identifying the safeguarding concerns
      • Even if no, how did you make this judgement to draw this conclusion?
    • Detail the steps to be taken once concerns have been identified

 

Ø Consider others in relation the assessment and provision of care

o Family members – This includes children as well as adults

o Carers – This could include children as well as adults

o Other members of the multidisciplinary team
