A reflective critical analysis of your prescribing practice during one consultation using a recognised consultation model demonstrating safe practice and within a framework of professional development. This will include reflective analysis and appraisal of your approach to a) Assessing the patient and taking an appropriate medical, social and medication history from all available sources. b) Interpretation of this and relevant investigations necessary to inform treatment options and a diagnosis including the seeking of support and advice when necessary. c) Achieving your prescribing decision considering your knowledge of the condition, disease progression and potential treatment outcomes. Having considered the mode of action and pharmacokinetics of the medication alongside information from your assessment and history taking to support your decision. d) Critical application, analysis and evaluation of appropriate national guidelines, local protocols and current evidence base to your decision. e) Critical application of your professional code, legal frameworks/legislation and ethical principles throughout the process. f) Reaching a shared decision with your patient. g) Considering the patient’s needs for review and monitoring including other members of the MDT where appropriate to meet the patient’s needs. h) The conclusion of your reflection should include an action plan for your continuing professional development identified through the application of reflective processes. i) A copy of the prescription and drug monologue should be included as appendices 1 and 2. No other appendices should be submitted.
