Please answer the following questions for the Statement of Qualifications following the instructions given. Attached is the duty statement and my resume for more information. A Supplemental Questionnaire (SQ) is required. The SQ must be typed in 12 point font, single-spaced, on letter sized paper, (8.5″ x 11″), and be no more than two (2) pages in length. Responses should be numbered, as well as complete, specific, clear, concise, and include examples. Resumes, cover letters, and statement of qualifications will not be considered a substitute for the SQ. The SQ should address the following questions:1.Describe how your experience, education, knowledge, and skills translate to the responsibilities outlined in the Duty Statement.2.Please describe a work-related project or task where you have worked independently, or as part of a team, to produce a final product for either internal management or external stakeholders.3.Describe your experience in dealing with individuals at all levels within an organization, please demonstrate your ability to communicate effectively.Job posting as follows:This position supports the California Department of Public Health’s (CDPH) mission and strategic plan by enhancing preparedness and response for emerging public health threats through serving as the emergency response communication and coordination hub; providing leadership and vision through planning and guidance; effective management and oversight of emergency preparedness funds; and providing good customer service.The Associate Governmental Program Analyst (AGPA) works under the direction of the Staff Services Manager I of the State Planning and Preparedness Unit. The AGPA assists in the development, implementation, and exercising of the Emergency Response Plan and other operational response plans for the California Department of Public Health (CDPH) and the Department of Health Care Services (DHCS). The AGPA coordinates with Federal, State, and local agencies involved in emergency preparedness and disaster response planning to ensure an integrated, multidisciplinary response effort within the Medical and Health Coordination Center (MHCC). The incumbent develops and maintains authoritative knowledge of the Standardized Emergency Management System (SEMS) and the National Incident Management System (NIMS), the State emergency response structure, local, State, and Federal emergency preparedness and response plans, resources, and authorities.
