Advice on completing the ethical considerations aspects of a programme of research

Consent

Informed consent must be obtained for all participants before they take part in your project. The form should clearly state what they will be doing, drawing attention to anything they could conceivably object to subsequently. It should be in language that the person signing it will understand. It should also state that they can withdraw from the study at any time and the measures you are taking to ensure the confidentiality of data. If children are recruited from schools you will require the permission, depending on the school, of the head teacher, and of parents. Children over 14 years should also sign an individual consent form themselves. If conducting research on children you will normally also require Criminal Records Bureau clearance. You will need to check with the school if they require you to obtain one of these. It is usually necessary if working alone with children, however, some schools may request you have CRB clearance for any type of research you want to conduct within the school. Research to be carried out in any institution (prison, hospital, etc.) will require permission from the appropriate authority.

Covert or Deceptive Research

Research involving any form of deception can be particularly problematical, and you should provide a full explanation of why a covert or deceptive approach is necessary, why there are no acceptable alternative approaches not involving deception, and the scientific justification for deception.

Debriefing

Debriefing is a process of reflection once the research intervention is complete, for example at the end of an interview session. How will participants be debriefed (written or spoken feedback)? Withdrawal from investigation

Subjective:

CC (chief complaint): A
brief statement identifying why the patient is here. This statement is verbatim of the patient’s own words about why presenting for assessment. For a patient with dementia or other cognitive deficits, this statement can be obtained from a family member.

HPI: Begin this section with patient’s initials, age, race, gender, purpose of evaluation, current medication and referral reason. For example:

N.M. is a 34-year-old Asian male presents for medication management follow up for anxiety. He was initiated sertraline last appt which he finds was effective for two weeks then symptoms began to return.
Or
P.H., a 16-year-old Hispanic female, presents for follow up to discuss previous psychiatric evaluation for concentration difficulty. She is not currently prescribed psychotropic medications as we deferred until further testing and screening was conducted.
Then, this section continues with the symptom analysis for your note. Thorough documentation in this section is essential for patient care, coding, and billing analysis.
Paint a picture of what is wrong with the patient. First what is bringing the patient to your follow up evaluation? Document symptom onset, duration, frequency, severity, and impact. What has worsened or improved since last appointment? What stressors are they facing? Your description here will guide your differential diagnoses into your diagnostic impression. You are seeking symptoms that may align with many
DSM-5 diagnoses, narrowing to what aligns with diagnostic criteria for mental health and substance use disorders.

Substance Use History: This section contains any history or current use of caffeine, nicotine, illicit substance (including marijuana), and alcohol. Include the daily amount of use and last known use. Include type of use such as inhales, snorts, IV, etc. Include any histories of withdrawal complications from tremors, Delirium Tremens, or seizures.

Current Medications: Include dosage, frequency, length of time used, and reason for use. Also include OTC or homeopathic products.

Allergies:
Include medication, food, and environmental allergies separately. Provide a description of what the allergy is (e.g., angioedema, anaphylaxis). This will help determine a true reaction vs. intolerance.

Reproductive Hx:
Menstrual history (date of LMP), Pregnant (yes or no), Nursing/lactating (yes or no), contraceptive use (method used), types of intercourse: oral, anal, vaginal, other, any sexual concerns

ROS: Cover all body systems that may help you include or rule out a differential diagnosis. Please note: THIS IS DIFFERENT from a physical examination!

Objective:

Diagnostic results: Include any labs, X-rays, or other diagnostics that are needed to develop the differential diagnoses (support with evidenced and guidelines).

Assessment:

Mental Status Examination: For the purposes of your courses, this section must be presented in paragraph form and not use of a checklist! This section you will describe the patient’s appearance, attitude, behavior, mood and affect, speech, thought processes, thought content, perceptions (hallucinations, pseudohallucinations, illusions, etc.)., cognition, insight, judgment, and SI/HI. See an example below. You will modify to include the specifics for your patient on the above elements—DO NOT just copy the example. You may use a preceptor’s way of organizing the information if the MSE is in paragraph form.

Diagnostic Impression:
You must begin to narrow your differential diagnosis to your diagnostic impression. You must explain how and why (your rationale) you ruled out any of your differential diagnoses. You must explain how and why (your rationale) you concluded to your diagnostic impression. You will use supporting evidence from the literature to support your rationale. Include pertinent positives and pertinent negatives for the specific patient case.

Also included in this section is the reflection. Reflect on this case and discuss whether or not you agree with your preceptor’s assessment and diagnostic impression of the patient and why or why not. What did you learn from this case? What would you do differently?

Also include in your reflection a discussion related to legal/ethical considerations (
demonstrating critical thinking beyond confidentiality and consent for treatment!), social determinates of health, health promotion and disease prevention taking into consideration patient factors (such as age, ethnic group, etc.), PMH, and other risk factors (e.g., socioeconomic, cultural background, etc.).

Case Formulation and Treatment Plan 

Includes documentation of diagnostic studies that will be obtained, referrals to other health care providers, therapeutic interventions including psychotherapy and/or psychopharmacology, education, disposition of the patient, and any planned follow-up visits. Each diagnosis or condition documented in the assessment should be addressed in the plan. The details of the plan should follow an orderly manner.

Company Background
International Gadgets (IG) is a multinational company of 1,300+ employees and over $4 billion in revenue. Headquartered in Manchester, New Hampshire, the company designs, manufactures, sells, and supports a variety of low- and high-tech business productivity tools.
IG has manufacturing facilities in Detroit, Michigan, and Shanghai, China, and sources component parts from suppliers in Vietnam, China, Brazil, and the United States. While IG maintains sales offices in New York, Chicago, Los Angeles, London, Munich, Paris, Moscow, and Brussels, its products are sold throughout the United States and all of Europe and in China through a partner firm. IG employs a direct sales force targeting its top 1,000 customers, and it also sells via its website. Sales offices also include service and support operations, managed separately from the sales teams. The company is considering expanding more directly into China and exploring the possibility of opening a sales office in Beijing.
IG’s current top-selling product is the Office Drone, which was designed for workplace monitoring. The Office Drone is a small, unstaffed aerial vehicle for indoor use that includes real-time video monitoring and a targetable built-in water pistol and reservoir. The Office Drone is marketed to management and security teams to break up water cooler conversations and other nonproductive behaviors among groups of workers.
IG has introduced several new products and experienced rapid growth during the past year, with many new employees in all departments. The management team has greatly expanded as well. It includes several first-time managers, and the team is experiencing difficulties functioning as effectively as it has in the past. Communications within and between all parts of the organization, external partners, and suppliers and customers are at serious risk of completely breaking down.
In a recent example, Detroit Manufacturing was unable to meet a product delivery deadline because of parts shortages from supplies. (The component parts originated in Brazil and had been seriously delayed due to a dockworkers’ strike at U.S. West Coast ports.) The manufacturing team believed they had informed the sales team of the delay, but the sales team indicated they had never received that information and ended up losing the customer to a competitor. In another example, European sales were impacted when Switzerland unexpectedly devalued its currency, impacting IG’s profitability. Revised profitability forecasts were provided by several of the European sales offices, but others did not understand the request from the corporate office because of how it was worded, and the delay in response led to unnecessary costs.

Introduction
This portfolio work project, an earned value management analysis, is based on your selected business or IT project. You may choose to base your assessment on the Revive LLC case study for the development of a new online employee orientation module, or you may continue to use a company of your choice. Both human resource (HR) and information technology (IT) employees, as well as contractors, will be utilized in this project.
Earned value (EV) is defined “the measure of work performed expressed in terms of the budget authorized for that work” (Project Management Institute, 2021, p. 275).
Project managers examine cost control, tools, and techniques that define the procedures by which the cost baseline may be changed. They engage in performance measurement to assess the magnitude of variations that occur during project execution. EVM is used to measure project performance by comparing certain predefined variables. Microsoft Project is a tool used to track planned cost versus actual cost, calculate the earned value, and forecast the effects of some cost changes.

Option A: Project Revive LLC is a consulting firm of highly charged and accomplished project managers whose main stream of revenue comes from managing the turnaround of failing projects. As a consultant with Project Revive LLC you will study, analyze, and manage a failed project from NearlyFree.com. NearlyFree.com is a fictitious company and the project documents for NearlyFree.com project scenario are linked below. The NearlyFree.com project was originally scoped to develop and deploy an automated web-based training system for new employees to reduce the workload and cut down personnel resources required to complete the new employee orientation (NEO) training.
As a veteran member of the Project Revive team, you already know the value of continually examining performance throughout a project to further define and update project data. Special attention to documenting and identifying problems is required in time to make appropriate adjustments. The earned value management (EVM) technique is often used in this type of analysis. Continue your analysis of the failed project by applying the EVM technique to examine performance based on what was accomplished.
Option A Resources:
Option B: You may choose a business of your own. It must include (but is not limited to) the following criteria:

Clearly indicate the title of the project in every assessment.

Describe how technology and informatics are used to support the interventions used in practice?
During bedside shift report, both day and night nurse gives a report on the patient’s medical diagnosis and history at the bedside. Technology can help track the staff to ensure they’re doing report at the bedside. Some facilities have the ability to track when a nurse goes in and out of a room or every time, they use hand sanitizer.
Describe how employing evidence-based practice guidelines improve patient outcomes?
Evidenced-based practice proves that processes work by already being implemented into practice. Utilizing bedside shift reports involves the patient in their care and decreases mistakes. Facts show that the patient has caught mistakes during bedside shift report and nursing has also caught mistakes by during report or checking all lines and drains.
What benefits and challenges have you experienced with (the integration of) information technology in your practice?
The benefit we’ve experienced is a decrease in mistakes made, which is a huge patient safety issue so that helps keep patients safe. This also helps nursing staff set eyes on their patient in case anything is off. Patients enjoy being a part of their care and can request not to have bedside shift report if not. Some challenges we’ve had are getting the buy-in from nurses. Nursing is so task-oriented, and shortcuts are more prominent with staffing being extremely thin.
What strategies did you, or could you, use to overcome these challenges?
Explaining the why is a huge thing for me. I also know that nurses often don’t understand why something changes, so explaining the why will give them the reason and meaning behind it. Getting your charge nurses behind and helping navigate the change helps get more buy-in from the nursing staff as well.

One evidence-based practice that my facility uses to improve patient outcomes is the sepsis protocol. We use a web-based algorithm called SPOT-Sepsis Prevention and Optimization of Therapy to detect patients at risk for sepsis. The SPOT system is triggered by a combination of factors including the patient’s vital signs and lab values. If a patient flags as meeting the algorithms SPOT criteria a notification is sent to the patient’s care team and the patient is to be assessed for potential intervention. Upon notification, the patient’s nurse or provider has up to 27 minutes to input data regarding organ function status, mental status, and whether the patient is on antibiotics (which indicates that at some point someone thought this patient may have an infection). That combination of data-some technology-based, some human assessment based-is used to determine if a patient is sepsis positive. If so they should receive intervention in the form of fluid resuscitation and antibiotics within one hour of identification.
The benefits of having an actionable sepsis monitoring system and protocol for intervention are evident in patient outcomes, as mortality increases with each hour without intervention. The adaptation of machine-based algorithms such as SPOT monitoring aid in timely treatment, which is important in preventing mortality and improving recovery among affected patients (Shadid et al, 2023).
The challenges we have faced with this system are few, except for the occasional annoyance of nursing staff, which occurs when a patient is either admitted for sepsis and receiving treatment and yet continues to flag for screening, or when those flags are explained by a reason other than sepsis, yet they continue to flag for reassessment as often as every six hours. This can cause some “alarm fatigue”, where nurses miss signs that the system is picking up accurately related to sepsis or worsening sepsis which would require intervention. We overcome this by having a sepsis coordinator, who is assigned to independently reassess these patients and determine their status. This coordinator also assures that those flagging positive for sepsis are indeed receiving all necessary parts of the protocols implementation bundle in a timely manner.
