Create an action plan and presentation for an at-risk patient population and propose evidence-based intervention(s) for the identified population.

For this assignment, students will create an action plan and presentation for an at-risk
patient population and propose evidence-based intervention(s) for the identified
population. The paper should be a minimum of 1000- 1200 words in length, not
including the title and reference pages. Additionally, students will present the
information to the class or clinical partner(s).

Explain the feasibility and cost-benefit considerations of your health care economic issue as well as three ways to mitigate risks to the financial security of your organization or health care setting.

Background and Context
As a master’s-level health care practitioner, you are expected to Define and consider a number of factors when supporting change by analyzing the feasibility of a new initiative or identifying an issue within your organization. For example, you must take into consideration the various types of risk (such as patient safety, physical plant, financial, or reputation), as well as the present and future value of the service line or economic opportunity you are invested in. Additionally, you must also balance your ethical and moral responsibility to provide quality care to patients and populations while protecting your organization’s assets and economic viability in the near and long terms.
Instructions
For this assignment, you will develop a 10-slide business PowerPoint for the economic issue you have been working with as your topic in the previous week. You will then present this PowerPoint using Kaltura and submit the video.
This presentation asks you to examine the feasibility and cost-benefit considerations of implementing a proposed solution for your chosen issue, as well as analyze ways to mitigate risks.
Be sure to address each main point. Read the performance-level descriptions for each criterion to see how your work will be assessed. In addition, note the requirements for document format and length and for supporting evidence. Review the assignment instructions and scoring guide to ensure you understand the work you will be asked to complete and how it will be assessed.
You may also wish to consult the Guidelines for Effective PowerPoint Presentations [PPTX] if you need additional guidance as you are assembling your presentation.
Overall, your assignment submission will be assessed on the following criteria:

Summarize the problem and the potential impact the health care economic issue has on you, your colleagues, your organization, and the community at large.

This is an opportunity to tell your story and what the initiative means to you as a health care practitioner. Remember, this is a presentation. So, you want to be engaging and persuasive in order to build support for what you will be proposing to do later in the presentation.

Explain the feasibility and cost-benefit considerations of your health care economic issue as well as three ways to mitigate risks to the financial security of your organization or health care setting.

Make sure to present at least an overview of the data and numbers you are basing your cost-benefit analysis on.
Touch upon the ways in which potential risks could pose a threat to the financial security of your organization or care setting while you are addressing the ways to mitigate risk.
Support your explanation with evidence-based research or scholarly sources.

Describe the changes or solution that you propose be implemented in order to address the economic issue.

Include the potential benefits of implementation to your organization, your colleagues, or the community at large.
Support your initiative with evidence-based research or scholarly sources.

Explain how your proposed solution is culturally sensitive, ethical, and equitable within the context of the community and health care setting it will be implemented.

Make sure that your changes or solution are not unfairly burdening or disadvantaging any specific groups.
Double check that your changes or solution do not pose any ethical issues and are not culturally insensitive.
Ensure that both access and cost are equitable across all groups in the community that the proposed solution will be affecting.

Convey purpose, in an appropriate tone and style, incorporating supporting evidence and adhering to organizational, professional, and scholarly communication standards.

If you need more guidance on recording a video with Kaltura, refer to the audio and video information in Tools and Resources for Using Kaltura to record your presentation.
Be sure to reflect on the readings and discussions you have completed so far and consider how the content of these activities might inform your approach to this assignment. Ask your instructor any questions you have about the assignment.
Additional Requirements

Length of Presentation: The PowerPoint presentation should be 10 slides in length and presented orally using Kaltura to record and share the video. The video itself should not exceed 10 minutes.
Number of Resources: Must include at least five scholarly sources to support your work and meet scholarly expectations for supporting evidence. Apply APA formatting to in-text citations and references in the PowerPoint.
Technology Used: You must record yourself presenting your PowerPoint using Kaltura and submit it for review. You must also submit the PowerPoint file for review.

Case Study: Sandra, a 47-year-old divorced woman, received a diagnosis of stage 3 ovarian cancer 4 years ago. Read and answer the reflective study. 1. What are some of the stresses on Sandra’s middle-aged sisters and their families?  2. What resources are available to manage these stresses and support the sisters while caring for their dying sister Sandra?  3. Describe Sandra’s feelings about dependency and loss of autonomy because she is unable to do her own activities of daily living any longer

Instructions: Read the following case study and answer the reflective questions. Please provide evidence-based rationales for your answers. At least 500 words, APA, 7th ed. must be followed. References within the last 5 years published. No plagiarism, please.
CASE STUDY: Caregiver Role Strain: Ms. Sandra A.
Sandra, a 47-year-old divorced woman, received a diagnosis of stage 3 ovarian cancer 4 years ago, for which she had a total hysterectomy, bilateral salpingo- oophorectomy, omentectomy, lymphadenectomy, and tumor debulking followed by chemotherapy, consisting of cisplatin (Platinol), paclitaxel (Taxol), and doxorubicin (Adriamycin). She did well for 2 years and then moved back to her hometown near her family and underwent three more rounds of secondline chemotherapy. She accepted a less stressful job, bought a house, renewed old friendships, and became more involved with her two sisters and their families.
Sandra developed several complications, including metastasis to the lungs. Then she could no longer work, drive, or care for herself. She had been told by her oncologist that there was nothing else that could be done and that she should consider entering a hospice. She met her attorney and prepared an advance directive and completed her will. She decided to have hospice care at home and, with the help of her family, set up her first floor as a living and sleeping area. She was cared for by family members around the clock for approximately 3 days.
Sandra observed that she was tiring everyone out so much that they could not really enjoy each other’s company. At this time, she contacted the Visiting Nurse Association (VNA) to seek assistance. Her plan was to try to enjoy her family and friend’s visits. After assessment, the VNA nurse prioritized her problems to include fatigue and caregiver role strain. Other potential problem areas that may need to be incorporated into the care plan include anticipatory grieving and impaired comfort.
 Reflective Questions

Describe key factors you considered in the formulation of a proposed HR solution.

Prepare a 3-4-page hiring plan with your recommended course of action and analysis for filling a difficult position in the provided scenario.
Recruiting supports an organization’s short-, mid-, and long-term human capital needs. It can be a very easy and straightforward process, depending on the region, market, and the field of work. In some fields, prospective employees are readily accessible, and the organization will have the ability to select among a broad group of well-qualified candidates. In other fields, generating potential applicants will be very hard as they are not present, or if they are present, the competition among businesses in the market will be a challenge.
The nature of work is so very different from job-to-job and organizational role to organizational role. Each is important and makes valuable but very different organizational contributions. The field of HR studies the nature of work to attract and select the best applicants, and HR professionals follow SHRM’s behavioral competencies to support their decision making and, in this assessment’s scenario, to find a successful resolution to a hiring challenge.
Introduction
Scenario
For this assessment, assume you are an HR professional working as an HR business partner for one of the most prestigious nonprofit scientific research centers in the United States. The center is large enough to have its own office of general counsel. Recently, one of the assistant general counsels departed for a job at the White House. The general counsel wants the finest and most talented attorney candidates who are practicing in the field of research in a nonprofit setting. You have announced the open position and have had 807 applicants. You chose a group of 12 candidates, all of whom were turned down by the supervisor (hiring manager). The general counsel’s office demanded to see all applicant resumes, and your HR director agreed. After they reviewed all the resumes, they said none were good enough. However, they provided no additional information to you about their preferred applicant qualifications.
Your Challenge
While you are the HR business partner to the general counsel, you have many other organizational clients, each with their own needs and expectations, and therefore a limited amount of time to give to this particular client. Given that you have failed to select appropriate candidates, you consider spending some money from your very tight budget to hire a recruiting firm, but that is not a guarantee of success. You are considering the following three options: 1) recommending to the hiring manager that she reconsider the 12 candidates you selected, 2) asking her to allow you to re-announce, possibly with some changes to the position vacancy announcement, or 3) going to the outside (expensive) recruiting firm in spite of your budget constraints. The option you recommend should offer the greatest return to the organization and its employees.
Instructions
Prepare a 3–4-page plan that states your recommended course of action. Along with your recommendation, include the following:

Analyze how your proposed HR solution to an HR challenge contributes to organizational goals and strategies.

Weigh the barriers the business partner will face with each of the three options.
Include the internal implications, if any, to reconsider the 12 candidates and how that could be done.
Consider the passage of time against the need for another attorney in the office as well as the impact of cost if you choose to go to an external organization.

Describe key factors you considered in the formulation of a proposed HR solution.

Include the ethical considerations for recruiting and hiring in this scenario.
Integrate material from your references that offer a theory-based relationship to inform ethical considerations.

Explain the role of the HR practitioner in advancing a proposed HR solution.

Briefly explain your role to influence the eventual outcome. Consider aspects of leadership, negotiation, and consultation.

Discuss the application of any SHRM behavioral competencies to the process of solving an HR challenge.

Which competencies are most directly applicable to a successful resolution of this challenge and why?

Additional ?Requirements
The deliverable for this assessment applies professional skills in HRM to workplace situations that you will likely encounter in your day-to-day work in HRM. As part of your learning, we focus on the development of effective professional communication skills for the workplace. Your assessment should meet the following additional ?requirements:

Introduction

As technologies and the health care industry continue to evolve, remote care, diagnosis, and collaboration are becoming increasingly more regular methods by which nurses are expected to work. Learning the ways in which evidence-based models and care can help remote work produce better outcomes will become critical for success. Additionally, understanding how to leverage EBP principles in collaboration will be important in the success of institutions delivering quality, safe, and cost-effective care. It could also lead to better job satisfaction for those engaging in remote collaboration.

Professional Context

Introduction

The focus of Assessment 4 is on how informatics support
monitoring of nursing-sensitive quality indicator data. You will
develop an 8–10 minute audio (or video) training module to orient
new nurses in a workplace to a single nursing-sensitive quality
indicator critical to the organization. Your recording will address
how data are collected and disseminated across the organization
along with the nurses’ role in supporting accurate reporting and
high quality results.

Preparation

As you begin to prepare this assessment you are encouraged to
complete the Conabedian Quality Assessment Framework
activity. Quality health care delivery requires systematic action.
Completion of this will help you succeed with the assessment as

you consider how the triad of structure (such as the hospital,
clinic, provider qualifications/organizational characteristics) and
process (such as the
delivery/coordination/education/protocols/practice style or
standard of care) may be modified to achieve quality outcomes.

This assessment requires you to prepare an 8–10 minute audio
training tutorial (with optional video) for new nurses on the
importance of nursing-sensitive quality indicators. To successfully
prepare for your assessment, you will need to complete the
following preparatory activities:

Select a single nursing-sensitive quality indicator that you see
as important to a selected type of health care system. Choose
from the following list:

Staffing measures.
Nursing hours per patient day.
RN education/certification.
Skill mix.
Nurse turnover.
Nursing care hours in emergency departments,
perioperative units, and perinatal units.

Conduct independent research on the most current
information about the selected nursing-sensitive quality
indicator.
Interview a professional colleague or contact who is familiar
with quality monitoring and how technology can help to collect
and report quality indicator data. You do not need to submit
the transcript of your conversation, but do integrate what you
learned from the interview into the audio tutorial. Consider
these questions for your interview:

What is your experience with collecting data and entering
it into a database?
What challenges have you experienced?
How does your organization share with the nursing staff
and other members of the health care system the quality
improvement monitoring results?
What role do bedside nurses and other frontline staff have
in entering the data? For example, do staff members enter
the information into an electronic medical record for
extraction? Or do they enter it into another system? How
effective is this process?

For this assessment, imagine you are a member of a Quality
Improvement Council at any type of health care system, whether
acute, ambulatory, home health, managed care, et cetera. Your
Council has identified that newly hired nurses would benefit from
comprehensive training on the importance of nursing-sensitive
quality indicators. The Council would like the training to address
how this information is collected and disseminated across the
organization. It would also like the training to describe the role
nurses have in accurate reporting and high-quality results.

The Council indicates a recording is preferable to a written fact
sheet due to the popularity of audio blogs. In this way, new hires
can listen to the tutorial on their own time using their phone or
other device.

As a result of this need, you offer to create an audio tutorial
orienting new hires to these topics. You know that you will need a
script to guide your audio recording. You also plan to incorporate
into your script the insights you learned from conducting an
interview with an authority on quality monitoring and the use of
technology to collect and report quality indicator data.

You determine that you will cover the following topics in your
audio tutorial script:

quality of care and patient safety.

Why do new nurses need to be familiar with this particular
quality indicator when providing patient care?

Collection and Distribution of Quality

Indicator Data
According to your interview and other resources, how does
your organization collect data on this quality indicator?
How does the organization disseminate aggregate data?
What role do nurses play in supporting accurate reporting and
high-quality results?

As an example, consider the importance of accurately
entering data regarding nursing interventions.

After completing your script, practice delivering your tutorial
several times before recording it.

Additional Requirements

Audio communication: Deliver a professional, effective
audio tutorial on a selected quality indicator that engages new
nurses and motivates them to accurately report quality data in
a timely fashion.
Length: 8–10 minute audio recording. Use Kaltura to upload
your recording to the courseroom, or provide a working link
your instructor can access.
Script: A separate document with the script or speaker’s
notes is required. Important: Submissions that do not include
the script or speaker’s notes will be returned as a non-
performance.
References: Cite a minimum of three scholarly and/or
authoritative sources.
APA: Submit, along with the recording, a separate reference
page that follows APA style and formatting guidelines. For an

Describe What is the cause of a ventricular septal defect?

Case 1
You are examining a 5-week-old infant and hear a systolic murmur at the lower left sternal border. You suspect a ventricular septal defect. The parents are concerned and have several questions.
Answer the following questions based on this scenario.

Describe What is the cause of a ventricular septal defect?
What problems will our baby have due to his VSD?
How does the defect create these problems?
What diagnostic tests will be ordered?
What treatment will our baby need?

Case 2
Mr. Delano comes in to discuss the results of cardiovascular diagnostic tests. He is a 61-year-old White man with no complaints. Past medical history: left hip osteoarthritis and gout; Past surgical history: none; Allergies: none known; Medications: allopurinol 100 mg by mouth once a day; Social history: one to two alcoholic drinks three times a week; smokes one cigar every other month; exercises for 1 hour six times a week; retired nurse—works per diem at hospital once a week; Family history; mother, father, and one bother with hypertension; one brother with dyslipidemia.
Physical exam: vital signs – temperature 98.5°F; pulse 60 beats per minute; respirations 20 per minute; and blood pressure 134/67 mmHg; height 180 cm; weight: 85.5 kg; body mass index 26.3; physical exam unremarkable.
See cardiovascular diagnostics table in the text on page 157. ( Dlugasch, L. & Story, L. (2019). Applied pathophysiology for the advanced practice nurse (1st ed.). Jones & Bartlett. ISBN: 9781284150452
Answer the following questions based on this scenario.

Which of the cardiovascular diagnostic values demonstrate dyslipidemia and risk for atherogenesis?
Interpret and describe lipoprotein subfractions and apolipoproteins results.
Which of the cardiovascular diagnostic values are in the cardioprotective range?
What lifestyle recommendations, such as diet and exercise will be made based on these results?
What is his ASCVD risk score? What parameters (e.g., blood pressure) are used to calculate the risk score?

Case 3
Compare the following two (2) scenarios.
Scenario 1: Mr. Fernandez is a 62-year-old man who comes into the clinic complaining of left lower leg swelling that started 5 days ago. He says the leg hurts but mainly when he walks and does not get better after rest. He denies any trauma or fever. He returned from an international trip 2 days before the swelling started. He is healthy except for hypertension, which is well controlled with a daily thiazide diuretic.
Scenario 2: Mr. Kraft is a 65-year-old man who comes in complaining of lower leg pain that he has had for the past 3 months. He says both his legs cramp up in his calves when walking. He sits down to rest and the cramps go away. He denies any trauma or fever. He has smoked 1 pack of cigarettes a day for the past 35 years. He has type 2 diabetes for which takes metformin twice a day.
Answer the following questions based on these two (2) scenarios:

In which scenario are the history and findings consistent with a venous disorder and which is consistent with an arterial disorder?
What are the most likely venous and arterial disorder diagnoses?
Describe the pathogenesis of these venous and arterial disorders.
What would be expected physical examination findings with a venous disorder and which with an arterial disorder?
Which diagnostic tests and treatment are indicated for a venous disorder?
Which diagnostic tests and treatment are indicated for an arterial disorder?
Professional reflection on the 3 cases and add citations and references.

What barrier(s) could impact the success of your funding partnership? and How will you work to overcome the barrier(s)?

PARTNER COLLABORATION BARRIERS
Some projects are best served by engaging in partnerships. There can be many reasons for partnering, but primarily it is because the need or problem requires the involvement of multiple stakeholders. In some cases, a funder may look at organizational capacity and question whether staff has the experience and skills to carry out the project successfully. Other grants, depending on the nature of the RFP description, will specify partnerships. And some projects are simply too complex and wide ranging to even consider within a single organization.
It is important to understand that the time to reach out to potential partners about opportunities to apply for grant funding is when a need is first identified at an organization. Those who reach out to brand new “partners” because they must in order to submit a proposal may find it doesn’t work once the project is implemented. The barriers to successful partnerships can vary from lack of communication to hidden agendas.
Please review the Partnerships: Framework for Working Together Guidebook PDF and consider the potential barriers that can impact the success of a Describe the potential barriers that could impact the success of your prospective grant partner.

BY DAY 3 OF WEEK 8

Post a 2- to 3-minute “Vlog” (video blog) reflecting on the following about barriers to successful partnerships:
· What barrier(s) could impact the success of your funding partnership?
· How will you work to overcome the barrier(s)?
· Provide an explanation of the importance of building a relationship between funder and grantee. Describe key elements of this relationship, providing a rationale for your thinking.
· Describe why the grant funder/RFP’s criteria are important and how they can influence the outcomes of the funding process.
· Address both a Memorandum of Understanding and Letters of Agreement that would be necessary.

Note: Please review the Kaltura Media Uploader section of the course on instructions on how to upload media to the classroom/discussion.

Be sure to support your analysis and conclusions with citations and references in APA format from the Learning Resources and your own research.
