Chomy was brought in by ambulance to the A&E after taking an impulsive overdose of 150mg paroxetine and 250mg of quetiapine, following multiple mental and social stressors. She is known to Mental health services with a history of psychotic depression and emotionally unstable personality disorder (EUPD). She was struggling to cope following her father’s death, who was her protective factor and she break-up of an online relationship recently, the termination of the home visit with her two young children, and the problem with her house utilities and outstanding repairs. She was experiencing poor sleep, poor appetite, self-neglect, and a lack of motivation to attend to Activities of Daily Living (ADL). She reported feelings of loneliness and isolation, and she does not know how she feels about the overdose or surviving the suicide attempt. The psychiatric liaison service (PLS) referral document suggested that due to her lack of social support and the interaction between her mental health illnesses and excess alcohol intake, Chomy was referred to the crisis resolution and home treatment teams (CRHT) for follow-up in the community to manage her current risks and with the view to refer onwards to the community mental health teams (CMHT), Triage, Assessment and Brief Intervention (TABI), Path 2 Recovery (P2R), and Wellbeing Service for, psychological therapies once stability has been achieved.

Scenario The Village Hall Slaugham Parish Council (SPC), the parish council for Slaugham village in Mid-Sussex in the south of England, has applied for and received a grant from central government to build a community centre to serve the population of the town and surrounding rural area. Site and Context SPC already owns the land on which the community centre will be sited; thus it is publicly-owned property. Give an explanation of what a building control officer will be looking for at one of the following construction stages: – i. Excavation of a strip foundation prior to the pouring of the concrete, or ii. Ground floor, Finished Floor Level (FFL) prior to the installation of the floor screed for a beam and block floor system, or iii. Pre-plaster, board fixing (tacking). (approximately 750 words) Task 3 – Health and Safety Once contracts for the construction of the community centre have been awarded, and the principal contractor is on site, the client wants to ensure that the various potential site hazards and risks to any visitors to the site are clearly and visibly communicated. For the client, design a poster reflecting the various hazards and risks for the Village Hall project during the construction phase. Base your poster’s content on your understanding of the site layout and proposed building construction, and your knowledge of the related Health and Safety Regulations. The poster shall include a combination of images and text. Images that are not your own will need to be referenced and all text needs to be input directly into the PowerPoint (or equivalent) file. Please avoid using images or scans that include embedded text, as this may compromise how Turnitin reads your file. (600 words equivalent) Additional Guidance For Task 1 Be sure to make use of the Town and Country Planning Act 1990, the NPPF and the Mid-Sussex District (Local) Plan, District Policy 39. Also use your Module Journal reflections from weeks 3, 7, 8 and 9 to help inform your response. For Task 2 Your primary source for the ‘Means of Escape’ review should be Approved Building Regulation Document B, Volume 2, Requirement B1, Sections 2 and 5. The ‘Definitions’ section and Appendices include critical information related to the accepted meaning of specific terms used in the document and interpretation of ‘Means of Escape’ and ‘exit’ -related clauses. Additional sources such as the Metric Handbook will also assist with interpretation of the ‘Means of Escape’ and ‘Escape Routes’. Tasks 1 and 2 should be submitted as a single Word (or equivalent) or PDF document in the first Tab of the Turnitin Submission Portal. The poster will not need to literally include 600 words, the actual word count will be considerably less, but should offer enough explanation to clarify the intent of images. Reference list and bibliography For Tasks 1 and 2 you should include a single reference list which includes a minimum of five separate, relevant and appropriate sources that you have written about and cited within your work. For Task 3, you should include a reference list on your slide which includes a minimum of two separate, relevant and appropriate sources that you have written about and cited within your work. A bibliography of uncited sources is not required.
