During your study of Block 4 you have considered various approaches to disseminating research findings and the approaches taken by different researchers. In Unit 22 you worked through a number of activities relating to designing a dissemination strategy. In this assignment you will build on the work you did in Block 4 in order to design and critically evaluate an appropriate dissemination strategy for a specific research project.

Suggested approach

In this assignment you will focus on both designing and critically evaluating a dissemination strategy for a study into what constitutes happiness from young people’s perspectives. The study explored aspects of young people’s feelings about happiness using a mixed methods approach to identify and address a range of issues.

You are provided with a reference to an external source reporting this research: Sutton (2021) and you need to use this article which you can locate and access using the search skills you developed in TMA 03. Use the OU Library search facility to first find the journal title, then use the information in the reference to navigate to the correct volume and issue in order to locate the article. Once you have located the article, download a copy to your own computer and read it.

The TMA task asks you to design and critically evaluate an appropriate dissemination strategy for the research project by Sutton (2021). As you read the article, make a note of aspects of the research and research findings you consider should be disseminated to the research community and also to others who might be interested in this research study. You should pay particular attention to dissemination strategies for the young people who took part in the research and how you might present the data to them in an informative, useful and ethical way. As you read, note other factors that you think may be of interest or relevance, for example, you may wish to read the research findings and recommendations in some detail and consider whether these are specific to the location, or the age group, where and with whom the research was undertaken or whether they might have relevance elsewhere. Ask yourself:

  • Why does research matter? (Be clear about how and why a particular piece of research is significant.)
  • Who are the audiences for the research? (Be clear the research can be important and relevant in all kinds of different ways to a range of different audiences.)
  • Who would be interested in the research findings and how they might they be made aware of them? Here it is important to consider the role of the young people who took part in the research and how (or if) the findings should be disseminated to them.
  • You should also keep in mind the ethics of dissemination. If you are disseminating widely, how will you ensure anonymity? If you are disseminating to a small part (such as those who took part in the research), how would you ensure confidentiality?

Before starting your TMA, you are advised to review your notes from Section 2 of Unit 22. Take time to consider who the potential audiences are for this study, which findings are relevant to which audience(s) and how might each audience be reached.

Although it is not necessary to find out about the researcher who carried out the project, you might be interested to know that this article was based on the work done for a recent PhD in Childhood Studies at The Open University. The article does not cover the actual ways the project was disseminated or any impacts it had. However that is not relevant to this assigment: you are being asked to think about how the results of this study could potentially be disseminated clearly, efficiently and ethically to all the the relevant audiences. Your assignment will be marked according to:

  • how well you are able to identify suitableaudiences for the research – who should hear about the findings of this research?
  • how realisticyour plans for reaching those audiences are – how might different audiences be engaged?
  • how well you think through the practicaland ethical problems involved in disseminating the research.

The assignment not only asks you to design an appropriate dissemination strategy but also to critically evaluate it. In order to do this, you will need to refer to at least two sources from the module material in reflecting on your strategy. What are the strengths and limitations of your strategy? How does each part of the dissemination strategy contribute to the whole? Is your strategy appropriate for the different audiences you identify?

The TMA builds on key material discussed in Block 4. These brief notes may be useful as you consider which module sources to use in your TMA. In Chapter 15 of the Module Reader, Robb focuses on the different types of discourse within childhood and youth research that reflect the different approaches researchers employ to share their work with a range of audiences across academic, policy and practice arenas. Robb highlights how research dissemination is an important consideration for researchers during the planning stages of their research. Getting research into policy and practice, however, is not a straightforward task. In Chapter 16 of the Module Reader, Ruxton, and in Chapter 17, Pascal and Bertram, provide detailed examples of the complex process of linking research with policy and practice and the different strategies that can be used for engagement and dissemination.

Although this study may appear somewhat removed from the small-scale study you will design in your EMA project, similar principles apply and you will be able to use understanding developed here in Part 3 of your EMA.

Suggested structure

There are many ways of structuring this assignment, including choosing whether to write an essay or a planning document, utilising sub-headings, tables, bullet points and other formatting devices. These suggestions do not have to be followed, but may be helpful if you are unsure how to approach writing this assignment.

Remember – An essay begins with careful and efficient planning. An essay plan helps you organise your ideas and design a strategy that communicates appropriate information to the relevant audiences.

  • In your introduction you could briefly describe the study described by Sutton (2021) and explain the main aims of your suggested dissemination strategy.
  • You could then explain the significance of the research and identify the audiences to whom the research findings are relevant. How and why is the research significant to different groups of research users? Who else may be interested in these research findings? What are the potential barriers to reaching particular audiences? Are there other potential difficulties to overcome?
  • You could then set out your suggested dissemination strategy. What will you be communicating to each of the audiences you have identified? How will you reach those audiences? Do not simply list and describe each element within the broader strategy, but provide details of howand why the different elements of the strategy will enable the results of the research to be communicated effectively and usefully to the different audiences. Consider the use of varied channels to engage with an audience and to facilitate communication and impact.
  • You also need to critically evaluateeach of the elements in your strategy. You might do this either when discussing the different elements of the strategy or in a separate section of your TMA. Think about the practical and ethical problems involved in implementing the strategy, and show that you are aware of the limits of what researchers can achieve.
  • Finally, in a short conclusion, summarise the aims of your dissemination strategy and reflect on how effective you think it would be in meeting those aims.

SECTION 1. HEALTH CARE ADMINISTRATION PROBLEM AND PROJECT SCOPE

1.1  Introduction

More than 100 million Americans rely on Medicare and Medicaid medical coverage in the United States. (Berwick & Gilfillan, 2021). Medicaid insurance development was to reduce the cost of Healthcare, primarily for middle-income earners with limited resources and income (Dickman et al., 2017). Additionally, the Medicaid and Medicare policies cover individuals, including personal care services and nursing homes. According to Ghosh et al. (2017), Medicaid is one of the largest sources of funding for middle and low-income earners and persons who may be suffering from any disability and cannot afford to pay for continued medical-related services. While Medicare’s primary aim is to provide healthcare services for people over the age of sixty-five or disabled, Medicaid provides health care services by reducing medical costs, especially for low and middle-income earners (Dickman et al., 2017). Research has shown that due to the many discrepancies present in these insurance covers, increasing the duration of coverage is likely to reduce the significance of these insurance policies (Dickman et al., 2017).

The Medicare guidelines provide for one hundred and ninety psychiatric days of coverage for patients, meaning if any patient exceeds their stay in the hospital beyond the allotted days, they will be required to pay for any additional costs they may incur. The lack of maximum coverage by any Medicare recipient increases the burden of Healthcare to the patient, which can be way beyond the patient’s financial capacity.

The lack of insurance policies maximum healthcare cost to lengthy hospital stays can increase the stress and anxiety levels of the patients and consequently reduce their recovery rate. The patient’s family and friends may become overwhelmed due to the financial cost needed to assist their loved one who may be in recovery in helping to promote their wellbeing. With such thoughts in mind, the Medicare and Medicaid process can be revamped to close the gap for unpaid medical costs due to increased hospital stay, thus promoting increased financial revenue at a Major Psychiatric facility in the New York Metropolitan area.

1.2 Capstone Topic

This Project will analyze the payment process at the psychiatric facility in the New York Metropolitan area. The Doctoral Project will shed light on how healthcare administrators see difficulties losing financial revenue relating to Medicare and Medicaid health coverages. This information will show how the gap for unpaid patient revenue is related to a hospital stay, allowing for increased financial gain for the facility.

The roles and responsibilities for healthcare administration include developing effective working schedules for staff members promoting effective communication, which provides for promoting staff training to effective management of patient fees and billing (Popejoy, 2016). Additionally, healthcare administrators ensure that the facility under their care complies with all the health laws and regulations and effectively manages the hospital finances (Rosenbaum, 2019). In this regard, healthcare administrators manage the Medicare and Medicaid process, which covers the portion of patient fees and billing related to their financial management. The Project analyses the payment process at the facility on Medicaid and Medicare health coverage.

It is part of my Doctor of Business Administration (DBA) program with a specialization in Healthcare Administration and advocacy.

The project analysis shows the behavior of the increased revenue losses reported at the psychiatric facility due to the Medicaid and Medicare payment process. More specifically, this facility has been significantly incurring losses whenever the patient’s Medicare days deplete revenue due to increased hospital stay, which is lost revenue related to the facility’s finances.  Additionally, the Medicaid health insurance only covers patients under twenty-one years and those above sixty-five years, especially at the significant psychiatric facility in the New York Metropolitan area, an article 32 facility. Any patients between the gaps not covered by Medicaid generate revenue loss for the facility, regardless of their income and financial status (Taylor et al., 2021). For these reasons, the doctoral Project will analyze policies and practices to ensure Medicaid and Medicare effectively bridge the financial gap present at the psychiatric facility. Closing the gap for unpaid Medicare and Medicaid days will show how this major psychiatric facility have been losing revenue over many years and what recommendations or information can help close the financial revenue gap. The Project aligned with the Healthcare Administration’s strategic changes through an extensive focus on improving the facilities’ financial growth strategy with a focus on their organization’s practices.

1.2.1 Problem of Practice

Medicaid is America’s most significant financial contributor to Healthcare through the Medicaid post-Affordable Care Act, providing care to more than seventy-five million Americans (Rosenbaum, 2019). In collaboration with the Affordable Care Act, Medicaid has had significant outcomes, including reducing mortality rates and reducing diseases, thus improving health care

(Rosenbaum, 2019). The Federal government in the Medicaid disproportionate hospital share (DSH), relating to regulated required payments, hopes to settle unpaid hospital care costs to better access Medicaid and patients without health insurance and income that can stabilize facilities safety-net. The merger between Medicaid and the Affordable Care Act has increased revenue collection in rural hospitals. The union allowed dentists and other medical practitioners can transverse through these areas to help marginalized the poorer regions and patients who live there to have access to dental and other care-related services (Mazurenko et al. 2018).

Hopefully, this practice can be seen in additional metropolitan areas also. Research has shown that the government spends over six hundred billion dollars on Medicaid health cover to ensure that the vulnerable, low and middle-income earners can access affordable, quality, and equitable healthcare services. (Derkyi-Kwarteng. (2021). However, despite the immensely positive change in the healthcare sector, the program has continuously encountered several challenges. One such challenge is a significant lack of innovation in ensuring the policy is effectively adapted from state to state in providing quality metrics compared to its effectiveness in different forms. The inadequate consistency in implementing the minimum standards on individuals and services that should provide has continued to paralyze the program’s functionality.

Additionally, the lack of good collaboration in promoting unison in improving Healthcare, particularly for the vulnerable and low income earning population, and increase in investment in terms of contracts through the federal government support in reducing the effectiveness of these health insurance. Derkyi-Kwarteng. (2021). Implementing a plan of action can improve or minimize the length of unpaid patient stays for an effective financial system.

Vujicic et al. (2021) noted that Medicaid has been insufficient in providing stable health coverage to people who become dependent on the service, regardless of their income levels. Therefore, implementation is needed to promote long-term investment in providing affordable and quality healthcare services with the limitation of Medicare days, which does not consider much for patients with chronic illnesses. With the challenge of the unpaid cost that accumulates after the said period depletes, there is a need for changes in the healthcare sector and its administrators in addressing such challenges created by the laws that govern such coverages cover. The development of Medicaid and Medicare has resulted in an exponential decline in uninsured people across The United States. Consequently, this phenomenon has increased demand for healthcare staff, increasing pressure to utilize available resources (Shaw, 2021). Additionally, the increased number of patients across The United States who cannot afford their medical bills due to the depletion or expiry of the insurance cover brings to light the financial loss of many hospitals and medical facilities. (Berkowitz et al., 2018).

1.3 Purpose of the Project

This doctoral Project will analyze policies and practices that can be implemented to ensure Medicaid and Medicare effectively bridge the financial gap present at the major psychiatric facility in the New York Metropolitan area, which incurs increasingly unpaid hospital stays. Several research papers have shown that Medicare and Medicaid have huge shortcomings in terms of innovation. (Mazurenko et, al. 2018). However, very few articles have explored the problem of financial losses incurred by hospitals occasioned by the failure of Medicare and Medicaid to take care of long stays in hospitals. As such, this paper will explore how to close the gap for Medicare and Medicaid payment process for unpaid patients’ length of stays, allowing for better Financial

Revenue for the psychiatric facility. However, the immense success of Medicaid and Medicare in providing affordable and quality Healthcare for all the American people does not overshadow its challenges that need addressing in conjunction with relevant health and government stakeholders. Despite many patients benefiting from the Medicare and Medicaid healthcare coverages, the confusion and limitations of the process have seen patients financially and emotionally stressed and stretched to incur the high medical cost, which leaves facilities like metropolitan psychiatric facilities in New York incurring huge losses (Alley et al., 2016).

The lack of maximum coverage by Medicare increases the healthcare burden to the patient beyond the patient’s financial capacity. The lack of insurance policies total healthcare cost to lengthy hospital stay increases the stress and anxiety levels of the patients and consequently reduces their recovery rate. The gains achieved through implementations of these covers have seen the development of other provisions and discrepancies that compromise their effectiveness. Strategies to close these insurance coverage gaps are critical in addressing these discrepancies that may ultimately lead to a health crisis.

1.3.1 Project Need

This doctoral Project ensures a mutual benefit between the patient and the hospital. While the patients holistically gain from the Medicaid and Medicare covers, the hospital also collects its revenue through service payment. There is a need for modeling payments processes of Medicare and Medicaid to cater for the cost of patients who are not within the age limit and those that exceed the period of care due to lengthy hospital stays. Resources for the Project include internal information obtained through expert financial staff members directly associated with the unpaid patient stays at the psychiatric facility in New York.

Having insight relation to the financial improvement for due patient days will provide the knowledge for this Project, as well as economic statistics showing loss of revenue for unpaid patient days and how it affects the major psychiatric facilities’ bottom line.

How can the Medicare and Medicaid payment process close the gap for unpaid patients’ length of stay, allowing for better Financial Revenue at a major psychiatric facility in the New York Metropolitan area?

What factors impact the level of disaggregation of data that would be required when performing substantive analytical procedures?

Case: Logan Self Storage Part I: Substantive Analytical Procedures Facts Logan Self Storage, Inc. (LSS) is a C-Corporation based in San Diego, California. With approximately 10% of the self storage market nationwide, they own 460,000 storage units of various sizes throughout the United States and reported 2012 rent revenue of $115,687,453.00. Because rents vary significantly by geographic location due to land constraints and market demand for self storage services, LSS has divided its operations into five segments based on geographic location. These include operations in the West, Midwest, Southwest, South, and Northeast. LSS has three types of storage units that it offers for rent: small units, large units, and commercial units. The small units are 10 feet wide by 10 feet deep (100 square feet) total and are commonly used by renters for storage of extra belongings which do not fit in their apartments or for storage of small items between moves. The large units are 10 feet wide by 30 feet deep (300 square feet) and are commonly used for vehicle or boat storage or for storage of most household belongings between moves. For the January 2022 instance of the module, the industry under study is the ready-made garment industry: you can select any part of the supply network that produces garments, or footwear, or materials such as the fibres are made into garments. Your work will be a 3,000 word report, submitted via Canvas. This assignment is worth 40% of the total grade for the module.

 

Background:

The ready-made garment industry forms an important part of the economy of some nations. It’s a vast, global undertaking… but its operation has accompanied by concerns and even scandals. From factory disasters and sweatshop conditions to forced labour where cotton is grown; from toxic emissions to the wasteful practices of retailers and consumers… our consumption of garments has a significant impact on society and the natural world.

There is a need for a better understanding of the impacts resulting from the production of garments. Only when this is done on the basis of evidence and good science can policymakers decide upon what parts of the industry to promote, develop and subsidise – and which to tax and discourage.

Your job, in this assignment, is to choose any one part of the supply network for garments and conduct a quantitative analysis, putting this a report to share your findings. For your quantitative analysis, select an appropriate methodology such as measuring the water footprint, measuring the carbon footprint, etc. It is a key requirement of this assignment that you present original work in which you make calculations on the basis of evidence.

Additional Guidance:The garment industry as a whole is far too complex for meaningful analysis. Instead, you should focus your attention upon a single facet of the industry where you can offer insights. The aim is to produce a worked example, using reasonable assumptions, to assess the environmental harm arising from the production (or harvesting, or processing) of the item(s) you have chosen. This may be a calculation of contrbution to climate change, or some other quantifiable mode of harm such as water use, worker safety, toxic emissions etc.

You should provide an overview of the organisations (or types of organisation) that work in the supply network for your chosen item, define appropriate boundaries for your study and discuss the practices that take place, presenting evidence and reasoning such that you quantify the impacts of supply chain activities.

Conduct original analysis using appropriate tools and methodologies. You might, for example, apply a life cycle assessment methodology such as PAS2050, using published conversion factors as a basis for your calculations, e.g. estimating carbon footprint based on the weight of an item, the distance and the modes of transport employed. Use the academic literature to find additional evidence of quantified economic, environmental and/or social harm.

Having reported this information, propose and justify a new strategy for the supply network: ideally a creative solution that changes such factors as the business model, the configuration of the supply network or the specification of key products of services. Cite the sources that gave you information about the sector as a whole, and about your chosen commodity. Use evidence to construct reasoned arguments that lead to workable solutions.
