This module includes a summatively assessed portfolio which contributes to your overall grade for this module. Your Student Portfolio should demonstrate your personal and professional development and be cross referenced to the four NMC Domains: • Professional values • Communication and interpersonal skills • Nursing practice and decision making • Leadership, management and team working 2 How to structure your Learning Profile: • Please use a different patient in sections 1 & 2 • Please include the word count for the relevant sections as indicated below • Please ensure you profile clearly identifies how each section maps to the NMC domains and includes a confidentiality statement. You may decide to do this as a covering statement and cross-referenced check list at the start of your assignment. • We strongly advise you to reference each section this enables you to keep track of your references and makes for easier reading, however, this is not compulsory and 1 overall reference list is acceptable Your learning portfolio will be structured in to five sections and contain the following evidence: Section One A timeline of a patient’s journey (not included in word count and can be presented as chosen by the student) and a 1000-word reflection using a model related to the care journey. Within your reflection you are expected to identify any areas where care delivery could have been improved/enhanced and to identify interprofessional working opportunities. Need to demonstrate level 6 reflective writing Section Two Identify a case study of an individual experiencing complex healthcare needs and complete a mapping of the pathophysiology related to their diagnosis. This should be supported by referenced sources and may include diagrams where they are fully explained. Maximum word count 500. Should return to the patient here, no limits to number of diagrams used should demonstrate some complexity in the written element. Section Three Identify a case example and complete a full care plan giving a rationale for your care decisions. (Should reference nursing model used, references needed here re 3 rationales) Nursing interventions should be clear and clearly identify patients need/ problem
Section Four
Identify 3 critical incidents within your placement experience in year 3 and complete an analysis related to these events. These events must be related to clinical practice and may include: a deteriorating patient; care of a dying person; unexpected death; medicines management; clinical risk assessment/management etc. There should be a brief description of the incident (maximum 100 words) followed by analysis (maximum 750 words) per incident. Should demonstrate level 6 analysis, can use a reflective model if you want to it can help with structure Section Five This section should include evidence of your engagement in clinical supervision by the inclusion of your supervision record sheet and a completed learning diary sheet related to each of your organised clinical supervision sessions. It is important to maintain anonymity and confidentiality throughout this documentary evidence. A maximum of 2 peer supervisions can be included and 3 clinical (5 total). References needed in this section as students are asked to write their reflections in more detail and use SMART goals using literature to support. Section in the reflective writing on what they are taking forward should be detailed to clearly demonstrate what you have learnt and are taking forward.
